The following form is intended for members of the class defined in the judgment of the Quebec Court
of Appeal.
Persons who were aged 18 years or over on December 21, 2000, who have suffered
harm arising from the contamination of the groundwater by trichloroethylene (TCE) and its breakdown
products originating on properties owned by the Government of Canada at DRDC Valcartier and the
Société immobilière Valcartier Inc. (now GD-OTS Canada Inc.), or from troubles and inconvenience
caused by aqueduct connection work, who resided in Shannon:
- from January 1996 to December 2000 in one of the residences connected to the aqueduct during the
September to December 2001 aqueduct connection phase (described in Appendix A
);
- in one of the residences connected to the aqueduct during one of the aqueduct connection phases
that took place from 2001 to 2006 (described in Appendix A
); or
- from April 1995 to March 2000, in one of the housing units supplied with water from the military
base’s aqueduct system (described in Appendix A
).
THE DEADLINE FOR SUBMITTING A CLAIM IS JULY 15, 2023.
A Claim consists of a form and supporting documents. A separate Claim shall be submitted for each
individual.
Claims must be submitted to the Claims Administrator electronically, by
no later than July 15, 2023, via the Claims Portal at actioncollectiveshannon.ca.

In the event it is impossible for
you to submit your Claim on the Claims Portal, and only then, you may request a paper form from the
Claims Administrator. If you are sending a paper form to the Claims Administrator, you must keep a
proof of mailing the Claim to confirm receipt (e.g. tracking number and delivery
confirmation).
Claimants (or their lawyers or mandataries) must inform the
Claims Administrator in writing of any changes in their contact information or
legal representative.
Please keep copies of all documents you submit to the Claims
Administrator.
CONFIDENTIALITY STATEMENT
The personal information provided in this form will be used solely for the purposes of this claims
process and will not be disclosed to third parties. However, the Claims Administrator, Class Counsel
and the Defendants may, without further formality, file with the Court this form or any other
document containing relevant personal information about the Claimants during the claims
process.
This statement is subject to the application of any law of public order, including
the access to information legislation applicable to the Government of Canada, or any other
disclosure requirements applicable to the Government of Canada.
